How to activate & sign in

- Enter the Member ID (from your UHC card), date of birth, and ZIP code.
- Create a username and a strong password and click Activate or Continue.

- After activation, sign in at myuhc.com or the UHC app using your new credentials.

- Click Forgot Username or Password?
